Living Wills

A living will governs the initiation, continuation, withholding or withdrawal of life-sustaining treatment.

It is for use when you have an end stage medical condition that leaves you unable to communicate with your doctor or you are determined to be permanently unconscious. An “end stage medical condition” is defined as:

"End-stage medical condition."  An incurable and irreversible medical condition in an advanced state caused by injury, disease or physical illness that will, in the opinion of the attending physician to a reasonable degree of medical certainty, result in death, despite the introduction or continuation of medical treatment.

A living will does not have to be notarized.
